Students with Medical Conditions
It is vital that the school is aware of any students with medical conditions so staff are able manage a medical emergency appropriately. Many students require emergency medication to be administered at school. In order for staff to administer emergency medication to students, certain forms must be completed by the student's prescribing physician each school year. All forms expire on June 30 and new forms must be completed and signed by the prescribing physician. The new form(s) and student's medication should be brought to the main office the week prior to the start of the new school year; please avoid waiting until the first days of school when it is often chaotic and very busy in the office. Forms related to specific medication requirements can be found below.
Administration of Medication by Injection (i.e. Epipen)
Administration of Oral Medication (i.e. inhaler, Benadryl)
Student Allergy Management Plan (i.e. anaphylactic)
Mental Health
